Licence Application Cover Form/Update of Licensee Contact Information

For a company / an organisation to
a) Apply for a National Authority (Chemical Weapons Convention)[NA(CWC)] licence or apply for amendments to their existing NA(CWC) licence required under the Chemical Weapons (Prohibition) Act, Chapter 37B and the Chemical Weapons (Prohibition) Regulations.

b) Inform the National Authority (Chemical Weapons Convention)[NA(CWC)] of any changes to the existing main / alternate NA(CWC) licence contact applicants.

The following information is required to fill in the form:
- Company / Organisation Unique Entity Number (UEN)
- Company / Organisation Details and Nature of Business
- Main and alternate licence applicants’ details and NRIC / Passport Number
- Letter of Authorisation (with company letter head) authorising the new NA(CWC) main and / or alternate licence applicants to handle matters relating to the NA(CWC) licence

Licence Application Form (Amendment)

For a company / an organisation to apply for certain amendments to their existing National Authority (Chemicals Weapons Convention) [NA(CWC)] licence required under the Chemical Weapons (Prohibition) Act, Chapter 37B and the Chemical Weapons (Prohibition) Regulations.

The amendments include the following:
- Addition of new Schedule 2, 3 chemical / Unscheduled Discrete Organic Chemicals (DOC)
- Addition of new activity for Schedule 2/3 chemical
- Increment of maximum aggregate for licensed Schedule 2/3 chemical / DOC

For a company / an organisation to apply for a National Authority (Chemical Weapons Convention)[NA(CWC)] licence for Schedule 2, Schedule 3 and / or Unscheduled Discrete Organic Chemicals (DOC) required under the Chemical Weapons (Prohibition) Act, Chapter 37B and the Chemical Weapons (Prohibition) Regulations.
